Underwood is a retired firefighter and paramedic who worked for the Caribou Fire & Ambulance Department. He lives in Presque Isle, Maine, with his fiancée, who is also an EMT. He has five children, nine grandchildren, and one great-grandchild.

Underwood is committed to lowering taxes, reducing regulations, and increasing local control. He is also a supporter of veterans, seniors, and rural communities.
